What Is Corneal Collagen Cross-Linking?
Just been told your keratoconus is progressing? CXL is the procedure your doctor will usually recommend first. It strengthens the collagen bonds in the cornea using riboflavin (vitamin B2) drops and a controlled dose of UV-A light, a preventive treatment whose whole job is to halt the disease.
How Does the Procedure Work?
The riboflavin soaks into the corneal tissue first. Then the UV-A light triggers a chemical reaction that builds new bonds between the collagen fibers, turning a soft, flexible cornea into a firmer, more stable one. To be clear: the goal is not to improve today's vision, but to stop it deteriorating.
Types of CXL Protocols
Conventional CXL, the Dresden protocol, takes 60-90 minutes and is still the gold standard. Accelerated CXL uses a higher light intensity to finish in 10-18 minutes, with equivalent outcomes in most studies. Transepithelial CXL leaves the surface epithelial layer in place; recovery is faster, but it is less effective.
Who Needs CXL?
Any patient with active, progressing keratoconus, meaning serial corneal maps show measurable change from scan to scan. Most candidates are between 14 and 35. The point is to act before the distortion reaches a stage where only a corneal transplant can help.
After the Procedure
Expect moderate pain and light sensitivity for 3-5 days. Vision may dip slightly at first, that is normal and temporary. In our clinic we then track stability with follow-up corneal maps every 6-12 months.
