What Makes Scleral Lenses Different?
Scleral lenses are large-diameter rigid contact lenses (15–24 mm) that rest on the white of the eye (the sclera) instead of directly on the cornea. The space between the lens and the cornea is filled with saline solution — creating a smooth, spherical optical surface in front of the distorted cornea.
Why Are They Best for Advanced Keratoconus?
They completely bypass the irregular shape of the cornea — light passes through their smooth surface and the saline solution, ignoring the distortion underneath. Wearing comfort is greater than small rigid (RGP) lenses, and they provide continuous corneal hydration.
Comparing Lens Options
Glasses for early stages only. RGP lenses offer good correction but moderate comfort. Hybrid lenses for moderate stages. Scleral lenses are the best visually and most comfortable option for advanced cases.
Do They Replace Surgery?
Scleral lenses don't halt the progression of the disease nor do they treat it — they are an excellent compensatory visual solution. Some patients live comfortably with them for years, while others ultimately require ring implantation or corneal transplantation.
